User Manual for Submitting Data Using the Staff Form
Purpose of the Form
This form is designed for staff to securely upload specific documents or requests to IMAC’s administrative department. Follow this guide for correct submission.
Step-by-Step Instructions
1. Open the Form
•Navigate to the provided URL and enter the provided password to access the “Protected: Staff Form”.
2. Fill in Your Details
• 社員番号(Employee ID): Enter your employee identification number.
• 氏名(Name): Type your full name (first and last).
• 所属会社名(Company Name): Enter the name of the OJT company.
3. Choose the Purpose of Your Submission
• Use the dropdown menus to specify the purpose of your file submission. Select the appropriate category based on your needs from the list below:
①有給申請 (Paid Leave Application)
②定期券の写し (Copy of Commuter pass)
*The deadline for the regarding month is the morning of the second business day. Submissions which crosses the deadline won’t be calculated & paid that month.
③定期券領収書 (Copy of receipt of Commuter pass)
*The deadline for the regarding month is the morning of the second business day. Submissions which crosses the deadline won’t be calculated & paid that month.
④通勤・交通費明細書 (Detailed Commuting Expenses Statement)
*The deadline for the regarding month is the morning of the second business day. Submissions which crosses the deadline won’t be calculated & paid that month.
⑤出張領収書 通帳の写し(Travel Expense Receipts, including meals)
⑥タイムシート (Timesheet)
*Only for those whose leader signs, stamps, or includes the project’s name in the timesheet.
⑦住所変更 (Address Change)
*Upload your Resident card’s picture with registered new address (front & back).
⑧在留カード写し (Copy of Residence Card)
*For those who just arrived in Japan, picture with address registered.
⑨通帳の写し (Copy of Bank Passbook)
*For those who just arrived in Japan, clear picture of your bank passbook to receive salary.
⑩雇用保険被保険者番号の写し (Copy of Employment Insurance Number)
*For those who changed job & came to IMAC.
⑪源泉徴収票写希望 (Request for Issuance of Tax Withholding Statement)
*For those who resigns before December.
⑫離職票発行希望 (Request for Issuance of Certificate of Resignation)
⑬退職証明書発行希望 (Request for Issuance of Certificate of Retirement)
*Application for退職届(Resignation letter)
⑭証明書に係る申請書(Certificate Application Form)
*Application form for 前年度源泉徴収票(Previous Year Withholding Tax Slip)・在籍証明書(Certificate of Enrollment)・所得証明書(Income Certificate)・その他(Others)
⑮健康保険証 (Health Insurance Card)
*Upload after receiving your/wife’s health insurance card.
⑯健康診断レポート通知(Health Check Report Notification)
*Only healthcheckup report
⑰就労証明書発行希望 (Request for Issuance of Certificate of Employment)
⑱その他 (Others)
4. Attach Your File
• After selecting the purpose, upload the relevant file(s) using the file upload field.
• Only 3 files can be uploaded at a time (upto 5MB each).
• Ensure the file format is supported (e.g., PDF, JPG, or PNG).
5. Add Additional Details (Optional)
• In the message box (メッセージ), you can include any additional comments, explanations, or instructions related to your submission.
6. Submit the Form
• Click the “送信する” (Submit) button to send your information.
Tips for Successful Submission
• Double-check all fields before submitting to avoid errors.
• Use accurate and up-to-date information.
• For sensitive documents, ensure they are scanned or photographed clearly and legibly.
• If you experience technical difficulties, contact the IMAC administration team for support.
*Please be aware of this in advance: Any request/document to be provided by IMAC will take 14 business days.
